Cystic nephroma of the kidney is a rare benign neoplasm that is clinically and radiologically indistinguishable from cystic partially differentiated nephroblastoma and cystic Wilms¡¯ tumor. Identifying a cystic nephroma from these neoplastic diseases, particularly with Wilms¡¯ tumor, is imperative since the therapeutic strategies differ in these diseases. While cystic nephroma only requires a total surgical resection of the tumor mass, it is strongly suggested to perform a total nephrectomy followed by chemotherapy for the treatment of Wilms¡¯ tumor. Since the majority of the renal neoplasm are Wilms¡¯ tumors, the possibility of cystic nephroma is often overlooked until a histological confirmation. Therefore cystic nephroma should be considered as a possible diagnosis in cystic renal mass. We recently experienced a case of cystic nephroma which was confused with cystic Wilms¡¯ tumor before operation. The cystic renal mass was removed by simple nephrectomy. Five months has passed without any local recurrence since the operation.
